Providence Health & Services RN Navigator Total Joint Replacement in Walla Walla, Washington

Description:

Providence is calling an RN Navigator Total Joint Replacement/Alternative Payment Model (Hospital Administration Dept, Day Shift, 1.0 FTE) to Providence St. Mary Medical Center in Walla Walla, Washington. In this position you will:

  • Plan, coordinate and provide joint expertise and consultation by overseeing the complete care of the total joint replacement (TJR) patient. Navigate patients and their care partners from decisions for surgery through 90-days post op, ensuring pre-op surgery preparation, individualized education and care planning, safe discharge transitions, and post-acute follow up

  • Maintain close, collaborative and consulting relationship with all disciplines who provide and/or support patients receiving TJR care (e.g., nursing, case management, therapies, pharmacy, emergency room providers, patient access, inpatient rehab, volunteers, primary care and post-acute staff, etc.)

  • Oversee best practice care delivery and maintain awareness of key performance metrics and appropriate management costs; coordinate interventions to prevent adverse event such as ED visits and hospital readmissions.

  • Provide key statistics to the O&SM Regional Service Line Director. Statistics may include: clinical outcomes, patient satisfaction/survey results, cost data, canceled surgeries, LOS, readmissions, denied days, etc

  • Provide leadership on patient experience and quality improvement teams to ensure success in APM participation and Center of Excellence (CoE) designation.

  • Develop/participate in public, professional education programs to further knowledge, expertise, and awareness regarding conditions that may lead to Orthopedic specialty interventions and/or current best practice protocols for total joint replacement care.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited RN Program

  • 2 years experience in acute/ambulatory care orthopedics settings. Professional Orthopedic specialty certification (or equivalent) required within 2 years of hire date

  • Current WA State RN Licensure required

  • Current valid driver’s license

Preferred qualifications for this position include:

  • Case Management experience.

  • Proven working relationship with physicians, clinic personnel, and medical center staff, with demonstrated success in relationship building.

  • Proven leadership in continuous performance improvement and change management; metric collection and reporting.

  • Experience in education program development and presentation.

  • Current knowledge of alternative payment models.

  • Bi-lingual or multi-lingual skills preferred.
